Player Release Request are only submitted in this format. Incomplete information will result in delays in processing your request. Make sure all required data is included. Requests received will be processed as quickly as possible and could take up to 7 days to process. Verbal requests for a Players Release will not be processed. Please remember that all requests are processed by volunteer staff.

This form sends an email to the LDGHA Registrar. A signed Players Release form will be mailed to your address entered in this form. Confirmation of your acceptance of LDGHA Players Release policy will be included in your email request.

Upon receipt of her OWHA Player Release Form from the London Devilettes Girls Hockey,

will not be eligible to return to the London Devilettes until after April 30, 2012

We understand that if the above-mention player has already taken a release at any point in the time form the LDGHA, then reruns to the Devilettes following an absence of one or more years and, at a later date, seeks a second release form the LDGHA, the condition of that release will be determine by the Board. We understand that returning to play in the LDGHA may be affected by the rules and regulations in affect at that time.

We also understand that, by seeking this release, we aware of the OWHA Regular Three(3) Par(E) which states,  “A player may be release from one organization to another only once during a season, unless a change of the player’s domicile has taken place or with special approval from the OWHA Registrar.” This policy is effective for all releases issued after Jun 24, 2002.
