Current LDGHA players wishing to obtain Permission to Skate (PTS) forms MUST complete and submit a request form. Requests received will be processed as quickly as possible, but will take a minimum of 7 days to process. Verbal requests for PTS forms will not be processed. Please remember that all requests are processed by volunteer staff. Please send the completed PTS form to LDGHA Registrar
